Patient Rights and Responsibilities

Clients are at the centre of everything we do.

Gateway Community Health Centre is committed to ensuring that client rights are respected in all programs and services. The Centre is dedicated to the principle of providing competent, professional services in a fair, equitable and sensitive manner to all clients without prejudice or discrimination.

  1. A client receiving our service has the right to be approached in an empathetic, kind, courteous and respectful manner and to be free from stigma, assumptions, mental, physical and financial abuse.
  2. A client receiving our service has the right to be approached in a manner that respects the client’s individual identity, beliefs, history, culture, ability, dignity and privacy and that promotes their autonomy.
  3. A client receiving our service has a right to be approached in a manner that recognizes the client’s individuality and that it is sensitive to and responds to the person’s needs and preferences, including needs and preferences based on disability, sexual orientation, gender identity, gender expression, marital or family status, education, socioeconomic status, ethnicity, race, religion, spiritual, linguistic, and cultural factors.
  4. A client receiving our service has the right to information about the service provided to them, to be told who will be providing the service and informed of the provider’s role.
  5. A client receiving our service will be recognized as part of the care team and has the right to participate in their assessment of care requirements and participate in the development, ongoing review, evaluation and revision of their individualized plan of care.
  6. A client has the right to give or refuse consent to the provision of any service.
  7. A client receiving our service has the right to raise concerns, or recommend changes in connection with the services provided to them and in connection with policies and decisions that affect their interests to the Centre, government officials, or any other person, without fear of interference, coercion, discrimination or reprisal.
  8. A client receiving our service has the right to be informed of the laws, rules and policies affecting our operation and to be informed through the website or in writing of the Client Complaint Policy 1-A-14.
  9. A client receiving our service has the right to their records and to have their records kept confidential in accordance with legislative requirements, including the Personal Health Information Protection Act, S.O. 2004, c.3 Schedule. A, and the Gateway Community Health Centre Personal Health Information Protection Policy 1-A-18.
  10. Families, caregivers and substitute decision makers will be treated with respect and seen as valuable members of the care team.
