Community Resource Support & Social Prescribing

Health and well-being are influenced by more than medical care alone. Factors such as housing, income, education, working conditions, relationships, and social connections all play an important role in our health. Gateway CHC’s Community team can help individuals connect with the resources, programs, and supports they need to improve their well-being and quality of life.

Our community team can help you navigate and move between services, connect with the right supports when you need them, and even support completing forms and applications. This may include connections to:

  • community mental health and addictions agencies
  • social services
  • education
  • child protection
  • justice and corrections services
  • legal services
  • food security programs
  • housing supports
  • income assistance

Our community team can also connect individuals to community programs, group activities, and workshops to:

  • strengthen social connections
  • reduce isolation
  • build confidence
  • develop skills

Social Prescribing

To support your health and wellness beyond medical interventions, our team of doctors, nurse practitioners, nurses, and other health providers may refer you to one of our community team to support a “social prescription.” Social prescribing brings together the medical and social model of health, and is a way of connecting people to local, community-based activities and services that match your interests, strengths, and what matters most to you.

A social prescription might include joining a seniors’ group, taking an exercise or art class, participating in a community garden or theatre project, accessing a Good Food Box to support food security, or volunteering at a local event. These opportunities can help you develop new skills, share your talents, build meaningful relationships, reduce stress, and feel more connected to your community.

We work with you to identify opportunities that support your needs and goals and help address barriers to participation, including financial, geographic, transportation, or accessibility challenges. Our goal is to help you access the right supports, build meaningful connections, and feel empowered to take an active role in your health and well-being.
