Primary Health Services

As a Community Health Centre following the CHC Model of Care, we work with individuals, families and communities to strengthen their capacity to take more responsibility for their health and well-being.

Health Care for the whole family

We value people-centred care.

At Gateway Community Health Centre, clients and their families are the centre of our health team. Our team respects the dignity, autonomy and rights of the individual and involves clients in the planning and provision of services.

Primary health care is the first point of contact a person has with the health system. Our interprofessional team of physicians, nurse practitioners, nurses, health promoter, social worker, crisis counsellor, dietitian and others collaborate to provide services and programs to our clients. Working together, this team responds to the factors that impact the health and well-being of our clients. They ensure that our clients receive the best possible care. Primary care includes the prevention, diagnosis, treatment and follow-up of various health conditions, including referrals to specialists and diagnostic services.
