Medication Use

GCHC recognizes that Medication Reconciliation is a client centred safety initiative.

Evidence shows that medication reconciliation reduces potential for medication discrepancies such as omissions, duplications, and dosing errors. At GCHC, medication lists are reviewed and reconciled at stages of transfer such as:

  • Admission
  • Following discharge from the hospital
  • Following care by a specialist/external provider as opportunities present, such as a general health review or renewal of medication prescriptions.
  • General health review.

Clients are requested to bring all medications including over-the-counter to their designated appointment. An appointment with the Physician or Nurse Practitioner is also required for prescription renewal.
