Q: How is care at a CHC different than at a regular family doctor’s office?

A: The way CHCs are set up, your family doctor has access to a support system that helps him, or her, connect you and your family with a more complete and more coordinated kind of interdisciplinary team.

Your family physician is working with an interdisciplinary team – and this team is your team.

At Gateway, your team includes physicians, nurse practitioners, nurses, dietitian, social worker, health promoter, community resource worker and an early years team that focuses its services on families with young children.

CHCs engage communities and stakeholders by working in partnership with organizations, agencies and individuals to provide services that meet local needs and to enhance and promote better health care in our community.
