Q: How is care at a CHC different than at a regular family
doctor’s office?
A: The way CHCs are set up, your family doctor has access to
a support system that helps him, or her, connect you and your family with a more
complete and more coordinated kind of interdisciplinary team.
Your family physician is working with an interdisciplinary team – and this team
is your team.
At Gateway, your team includes physicians, nurse
practitioners, nurses, dietitian, social worker, health promoter, community
resource worker and an early years team that focuses its services on families
with young children.
CHCs
engage communities and stakeholders by working in partnership with
organizations, agencies and individuals to provide services that meet local
needs and to enhance and promote better health care in our community.
A:Our
Primary Care services are offered to individuals living in our Catchment area.
If you are seeking a doctor or need access to
services provided by the interdisciplinary team and wish to
become a client of Gateway, we invite you to come in and register with our
receptionist and your name will be added to our waiting list.
Q: Do I have to be a client to access all Gateway’s services?
A: No.
We have many programs that are offered to the
community that you can benefit from without being an enrolled client of Gateway.
An example of these services are:
The Early Years Program – programs and services offered
to families with children 0-6 years of age. For more information

Community Resource Program – Provides personal and practical support to persons seeking assistance with short-term situations such as housing, financial problems, food crisis, and assist those who are in need of assistance in the referral process.
Nutrition Counseling and
Education – Gateway has a Dietitian and a Diabetes Nurse who run diabetes
educational classes that can be accessed by anyone in the community.

A: Our Catchment Area is defined as:
The community served is the Municipality of Tweed, the Hamlet of Roslin, and
those portions of the Municipality of Centre Hastings previously known as
Huntingdon Township lying east of Highway #62 and south of the Village and
Township of Madoc.
