Better Care – Healthier Communities

 

CHCs deliver Primary Health Care that builds healthier communities across Canada.  The CHC Model of Care provides a setting where, under one roof, clients receive high quality care from a interdisciplinary team of professionals.  If health problems are triggered by social, environmental or economic problems in the community, the Centre responds by looking at the root causes that contribute to poor health. Because they partner with other social service sectors and harness the expertise of local community members, CHCs are effective in providing care to populations who have traditionally faced difficulties accessing care. CHCs are dedicated to the Eight attributes of Ontario's Community Health Centres.

Gateway's Catchment Area

 

The community served is the Municipality of Tweed, the Hamlet of Roslin, and those portions of the Municipality of Centre Hastings previously known as Huntingdon Township lying east of Highway #62 and south of the Village and Township of Madoc.  (Map of Service Area)

 

Gateway Community Health Centre (GCHC) is a non-profit, community-governed health organization serving the Municipality of Tweed. GCHC is committed to a wellness approach to primary health care delivered by a health care team comprised of Physicians, Registered Health Professionals including Nurse Practitioners, Nutritionist and Social Worker. Primary and community services such as health promotion and community development are also offered throughout Central Hastings.

We view health as the physical, emotional, psychosocial, economic and environmental well being of people within the family and within the larger community. We believe that the responsibility for achieving good health belongs to the individual and/or their family. It is the responsibility of health professionals, along with the community and government to ensure that adequate resources are available and accessible to meet the full range of health needs. We are committed to working with other agencies to increase awareness, knowledge, and understanding on a wide range of health-related issues.

 

We encourage teachers, service providers, service clubs, church and community groups, and individuals to participate in our health promotion programs.
